Enrolling in the Wayne County School System

  • The Wayne County School System, in its enrollment procedures, requirements and process does not and will not discriminate based on race, color, national origin, or immigration status.

    Registration will need to occur at the school that your child is zoned to attend.

    Documents that will be requested when enrolling students: 

    • Age Verification-the school system requires evidence of the student's date of birth and accepts as evidence a birth certificate or an alternate document from the prioritized list in the state enrollment rule.

    • Social Security Card or Waiver- (Parent may elect to sign a form at the time of registration stating the individual does not wish to provide the social security number, pursuant to O.C.G.A. 20-2-150.)

    • Immunization Certificate-the school system requires proof of immunization as required by O.C.G.A. 20-3-771, which includes an exemption for religious grounds and provisions for a temporary waiver.

    • Certificate of Eye, Ear & Dental Screening (DHR Form #3300)-this certificate is required to be issued to parents of children entering the first grade or the first time in a public school in Georgia.

    • Proof of Residency in parent/guardian’s name: (any one of the following or an alternate document from the list in the state enrollment rule.) 

      • Current utility bill (land line telephone, electric, gas, water, or cable) that includes the physical address of the residence; or

      • Current lease/rental agreement;

      • Current residential property tax statement or bill;

      • Current homeowner's insurance policy    
    • Any custody documents, if applicable.

    • Any restraining orders or other legal documents specifically limiting the access of any individual to the student(s) being enrolled.  

    • Documentation of any health concerns or allergies of which the school should be aware. 

    Documentation for Homeless students-Homeless students, as defined by the McKinney-Vento Act, shall be enrolled immediately with full participation in school activities, regardless of whether required documentation can be provided at the time of enrollment. The designated employee responsible for care of homeless students shall assist the person enrolling the homeless student or the unaccompanied youth in acquiring the necessary documents for enrollment in accordance with the requirements of the state enrollment rule and the Act.

    • Student Enrollment Form (one per student)

    • Student Health Form (one per student)

    • Request for Records/Transcripts (one per student)

    • Occupational Survey (one per family)

    Additional forms that may be used for enrollment:

    • Caregiver affidavit

    • Religious vaccination exemption

    • Proof of residence clarification

    • Social security exemption

    If registering for Pre-K, you will need to pick-up the additional required forms at the school.
